Location: Lymington (community-based role)
Hours: 22.5 hours per week
Salary: £27,500 per annum (FTE: 45K Per annum)
About the Role
A leading charitable palliative care provider is seeking a compassionate and experienced Clinical Nurse Specialist to join its community palliative care team. You will support patients with advanced, life-limiting conditions and their families, delivering high-quality holistic care across home and community settings.
Working closely with multidisciplinary teams across community and hospital services, you will play a key role in ensuring patients receive timely, personalised, and dignified care.
Key Responsibilities
- Deliver holistic palliative nursing assessments and hands-on care in community settings
- Develop, implement, and evaluate person-centred care plans in partnership with patients and families
- Provide responsive care, including urgent clinical interventions when required
- Offer emotional, psychological, and spiritual support to patients and families via visits and telephone support
- Promote patient choice, including preferred place of care and end-of-life preferences
- Act as a clinical resource and educator for health and social care professionals
- Work collaboratively within a multidisciplinary team to ensure seamless care delivery
- Provide expert advice and clinical support to community-based care teams
- Maintain accurate clinical documentation and contribute to audits and quality improvement initiatives
- Work flexibly across the service to meet patient and operational needs
About You
We are looking for a nurse who combines clinical expertise with compassion and sensitivity. You will have:
- Current NMC Part 1 or Part 2 registration
- Proven experience in palliative care, end-of-life care, or oncology
- Experience delivering care in community or home-based settings
- Full UK driving licence with at least 3 years’ driving experience (essential for community visits)
- Excellent communication skills with the ability to support patients, families, and colleagues with empathy and clarity
- Strong organisational skills, IT literacy, and the ability to manage complex situations
- A professional, compassionate, and proactive approach to care delivery
